Immunization Information

 

Immunization Self-Report

Newly entering undergraduate students at MSU who are in programs or classes taught on the East Lansing campus are required to complete the Student Immunization Self-Report Form on-line at immunize.msu.edu.

The form is completed by either reporting current immunization status or selecting a waiver specifically declining to give this information.

Submission of the form also acknowledges that the student has read and understands the Centers for Disease Control and Prevention (CDC) recommendations for vaccination of college-age students.


Failure to complete this requirement will result in an administrative hold being placed on the student's account that will prevent registration and enrollment at MSU.

MSU's recommendations are consistent with the most recent recommendations from national health authorities. You may visit their websites for more information.
